Athletic physicals for the 2019-20 school year will be given at Westview High School on TUESDAY, APRIL 16, 2019, for Westview students in current grades 5-11. Please complete all parts of the forms and make sure they are signed by both the student and the parent. Completed and signed forms, along with payment, should be turned into the school by WEDNESDAY, April 10, 2019. Unsigned forms CANNOT be accepted. Students can pick up forms from the athletic office. The cost for physicals is $20.00 and checks can be made out to WHS.
If you are going to your personal physician for an athletic physical, please use the athletic form from Westview. Physicals done by your doctor must wait until after April 1, for the next school year. Please enter the grade your child will be in for 2019-20 school year on the form.
