Athletic Physicals for the 2020-21 school year will be given at Westview High School on TUESDAY, APRIL 14, 2020 for Westview Elementary students @ 8:30 A.M.
Students need to pick up forms ( physical, cardiac & concussion) from office.
The cost will be $20.00 & checks are to be to Westview High School.
ALL FORMS must be completed by parent & student and returned with payment to Westview Elementary by WEDNESDAY, April 1, 2020
NO FORMS WILL BE ACCEPTED ON THE DAY OF EXAM.
Unsigned forms WILL NOT be accepted.
Please enter the grade your child will be in for 2020-21 school year on the form.
Reminder: All students must have a new athletic physical every school year to participate in athletics.
Physicals done by your family doctor must wait until after April 1 to be done for the new school year 2020-21.
